Appointments

To make an appointment, please call us at (865) 546-0157 or have your physician send a referral.

If you’re a new patient, you will receive paperwork in the mail to complete and bring with you to your first appointment. Please arrive 30 minutes prior to your scheduled visit.

Please notify our office as soon as possible if you are running late to your appointment. If you arrive late, we will try our best to see you as soon as possible. Depending on how late you arrive for your appointment, we may have to shift your appointment towards the end of the day. You may also choose to reschedule your appointment for another day.

Our office hours are Monday-Thursday, 8 a.m. – 5 p.m. and Friday, 8 a.m. – 12 p.m.

Please bring the following to your appointment:

  • Current insurance card
  • Valid government-issued photo ID
  • Any medications you are currently taking in their original containers
  • New patient paperwork
  • If you have been referred to our office by another healthcare provider, please contact our office prior to your visit to make sure all necessary paperwork and referral information has been provided.

Physician Referrals

If your insurance provider requires a physician referral to our practice, please bring your referral authorization with you to your appointment. You can also request that your referring physician send the authorization to our office prior to your visit. Unfortunately, we will not able to see you if we do not have your authorization at the time of your appointment.

Accepted Insurances

We accept many insurance plans. Click here for a complete list. We will request co-pay payments at the time of your visit. Be sure to let us know if your insurance coverage changes.

If you need insurance forms completed, please note there is a $25 fee for this service, which is due when you pick up the paperwork.

Prescription Refills

If you need a prescription refill, please contact your pharmacy first. They will work directly with us to refill your script. Please note it may take up to 72 hours to complete your prescription refill.
